Dr. Hammond always had a vision to find innovative ways to improve primary care. In 2006, he teamed up with CU students, Stacey Halvorsen (Hammond), Felicia Zeeh (Montano), and Matthew Zald who were participating the INVST Community Leadership Program, to create and establish the Colorado Center for Primary Care Innovation.
The Colorado Center for Primary Care Innovation became a 501c3 nonprofit organization in 2007 with a mission to facilitate primary care demonstrations of cutting edge ideas through a partnership with Westminster Medical Clinic. The goal of the non profit was to improve chronic disease management and ultimately find ways to empower patients to improve their health behaviors, their self-management skills, and ultimately their clinical outcomes.
Since its establishment, the Center has worked to design, implement, and study initiatives; driving new ideas in primary care and finding dynamic ways to improve patient care in our local community and beyond.
