1.) Sign into your digital account.
2.) Search for Colorado Center for Primary Care Innovation by name or by entering BD172 here .
3.) Click Enroll. Colorado Center for Primary Care Innovation will be displayed in the King Soopers Community Rewards section of your account. If you need to review or make changes, you can do this under your Account details.

*To participate, you must have a registered King Soopers or City Market loyalty card account to link to your organization. If you do not have a King Soopers or City Market loyalty card, they are available at the customer service desk at any King Soopers or City Market.
